SPRING SPORTS PHYSICALS FOR ALL STUDENTS (GRADES 7-12) WILL BE HELD ON: February 15, 2021 at the High School. The PIAA requires one comprehensive physical per year for eligibility to participate in interscholastic athletics. The PIAA requires that the date on the PIAA Physicians Form Section 6 must be no earlier than June 1, 2020 for the current school year. If the date on the form is incorrect, the physical will be returned, and the athlete will be ineligible to participate until the physical is dated properly. The Athletic Department does not require any immunization information as is stated in the physical packet. They will not accept any physical paperwork that is not on the PIAA and SHALER AREA forms can be found at the following links or may be picked up in the Athletic office at the High School or the Activity Office at the Middle School.

The fee for all physicals taken at school is $10.00 and is due at the time of the Physical. There will NOT be a Functional Movement assessment offered for spring sports physicals. Checks are preferred and can be made out to “S.A.S.D.”
ALL PHYSICAL PACKETS MUST BE FILLED OUT COMPLETELY AND SIGNED BY A PARENT OR GUARDIAN TO RECEIVE A PHYSICAL.
Your report time for the physical is based on the first letter of your last name, so we do not have people waiting for long periods of time and we are able to practice social distancing. Be prepared for the process to take an hour. You will not be permitted in the school before or after your scheduled time. A mask will be required to enter the building. See the time schedule below:

Letters K-O are at 9:45 am
Letters P-T are at 10:00 am
Letters U-Z are at 10:30 am
Someone will meet you at the front door of the High School. You will be given a daily screening to clear you from any COVID 19-symptoms and you will be escorted back to where the physicals are taking place. Again, you must have a mask to cover your mouth and nose and please do not report before your scheduled time.
