The Shaler Area School District, in partnership with Giant Eagle Pharmacy, will be hosting a Community COVID-19 Vaccination Clinic for any adult and student age 12 and older. There is no fee or copay required to receive the vaccination.
The Pfizer-BioNTech COVID-19 Vaccine will be offered on Tuesday, June 15, 2021, from 4-6 p.m. in the Shaler Area Middle School cafeteria, located at 1810 Mt. Royal Blvd., closest to the Shaler North Hills Library. This is a two-dose series with the second dose given 21-42 days after the first dose. The second dose will be offered at the same location and appointment time on Tuesday, July 13, 2021.
On the day of the Vaccination Clinic, community members and students are to bring with them:
- Completed immunization administration record form. Students younger than 18 must have a parent/guardian complete and sign the form. Please complete the form in its entirety and sign where indicated: COVID-19 Vaccine Immunization Administration Form for School Clinics. Students age 12-15 must be accompanied by a parent/guardian.
- Insurance card (If an individual does not have health insurance, they will still receive the vaccination at no cost. Individuals without health insurance must be able to provide the last 4 digits of their social security number.
- Photo ID (driver’s license or school issued ID)
