High School to host COVID-19 Vaccine Clinic for students age 16+

Dear Shaler Area High School families,
 
The Shaler Area School District, in partnership with Giant Eagle Pharmacy, will be hosting a COVID-19 vaccination clinic for students age 16 and older who are attending Shaler Area High School. There is no fee or copay required to receive the vaccination.
 
The Pfizer-BioNTech COVID-19 Vaccine will be offered on Thursday, May 6, 2021 during the school day. Cohort C students are invited to participate from 2:45 p.m. - 3:45 p.m. This is a 2-dose series and the second dose is given 21-42 days after the first dose. The second dose will be offered at school on June 3, 2021 during the school day. Cohort C students are invited to participate from 2:45 p.m. - 3:45 p.m. 
 
Parents/guardians of current Shaler Area High School students who wish to have their child vaccinated, must: 
1.     Complete the registration form by noon on Tuesday, May 4, 2021: https://forms.gle/N6gBz5LrXqvAD3vG9
2.     Print out and complete the immunization administration record form in its entirety, checking the consent box, and initialing: COVID-19 Vaccine Immunization Administration Form for School Clinics. Copies of the form will be available in the high school main office for families that do not have access to a printer. 
 
On the day of the Vaccine Clinic, students are to bring with them:
1.     Completed immunization administration record form (Students younger than 18 must have a parent/guardian complete and sign the form)
2.     Student’s insurance card (if a student does not have health insurance, they will still receive the vaccination at no cost)
3.     Photo ID (driver’s license or school issued ID)
