Students 16 yrs. & older
Portions of the work permit application will be completed by the secretary when the application is requested.  Proof of age is required (a birth certificate, baptismal certificate or passport are the most common forms).  The permit requires a parent signature.  When complete, a work permit will be issued.

Students 14 & 15 yrs. (same as above)
In addition a 14 or 15 year old student must complete a “Parental Acknowledgement of Minor’s Duties and Hours of Employment” form.  This form has a section to be completed by the employer (*a job must be in place) and a section for the parents to sign.

Students Under 14
Minors under 14 years of age may not be employed or permitted to work in any occupation, except children employed on farms or in domestic service in private homes. No minor under 14 years of age may be employed on a farm by a person other than the farmer. Under certain restrictions, caddies may be employed at the age of 12, news carriers at 11 years of age, and juvenile performers in the entertainment field.
