It is that time of year again!  If your student is going on our trip to Chicago this year, it is important that you complete the medical form!  A copy of the medical form as well as a letter from the Nurses can be found under forms to the right of this page.

This medical form is separate from the form that you completed for the Fall Marching Season.  For first time trip people, this form is necessary and must be as complete as possible.  It is notarized so that the nurses who travel with the Band have your approval since we will be caring for your child outside of Pennsylvania where we are licensed.

Please take your time and complete the form completely  

Important facts:

• We need TWO NOTARIZED COPIES WITH STUDENT PICTURES
• Notaries will be available at the Pancake Breakfast March 16.  This is a FREE  service that is provided by wonderful band parents who are also notaries.  
  
  • As with all medical forms, the information is private and will only be shared with the nurses.  The exception is allergies!  The chaperones and staff will have a list of all medication and food allergies.
• The deadline to receive the completed and notarized form is Friday, March 22, 2019.   We can update any additional information at Performance Night if necesssary.

If you choose to use your own notary, that is fine.  We will be collecting forms at the Pancake Breakfast or you can send them to:
