1) 1 dose of Tdap (Tetanus/Diphtheria/Pertussis) vaccine and 1 dose of Meningitis vaccine must be given to students prior to the start of 7th grade. Documentation must be provided to the School Nurse.
2) Dental Exam—Completed form to be submitted to the School Nurse at some point during the course of the school year. School Dental exams are provided for students in April/May upon parent request.
3) Scoliosis Exam—Completed form to be submitted to the School Nurse at some point during the course of the school year. Exam can also be completed by the School Nurse during Feb/March if private exam isn’t performed.
